Information About Sharon's Coaching and Counseling Practice
CONFIDENTIALITY:
The content of all professional interactions in this practice will be held in the strictest confidence unless this confidentiality is waived by you. This is a matter of professional ethics and integrity as well as a legal mandate. However, confidential information of this nature can, at times, be ordered to be revealed within an appropriate legal setting by court order. Also, information concerning current child abuse or child molestation and information concerning physical violence to others is required by law to be reported to the designated authorities. Therefore, I cannot legally hold this type of information in confidence.

APPOINTMENTS:
Sessions are 50 minutes. Any session that extends beyond 50 minutes will be charged on a pro-rated basis. A fee is charged for reviewing written materials or other relevant consultations.
Payment arrangements are made in advance with Carol Selby, my practice manager. I accept debit cards, Visa, MasterCard or a retainer.
It is my policy to charge in full for appointments that are not canceled with 24 hours notice.
I consider all appointments made with me as your commitment to the time scheduled. When I have sufficient notice, I can reschedule the appointment time I have committed to you.
INSURANCE:
Coaching does not qualify for healthcare insurance. Coaching that relates to business matters may be tax deductible as a business expense.
Counseling may qualify for healthcare insurance. I do not take responsibility for collecting your insurance. That is between you and your insurance company. I do not participate in any preferred provider or other managed care plans. If you have any questions about this, please ask my practice manager.
I have chosen not to be a Medicare provider and do not accept Medicare assignments. If you have Medicare insurance, please notify my practice manager. You will need to sign a Private Contract waiving your Medicare benefits for my services.
Upon your request, my practice manager will provide you with an insurance statement that will include all the required medical information necessary to process your insurance claim. You can then complete the general information section of your insurance claim form, attach my statement and mail it directly to your insurance company or its representative. If you are submitting insurance, there will have to be one identified patient and that person will have to receive a psychiatric diagnosis for insurance purposes. |
